OUT-OF-NETWORK INSURANCE DISCLAIMER

At this time, Pure Connections Therapy is a private pay practice and is considered out-of-network with ALL insurance providers. We are happy to provide a superbill (itemized receipt) upon request, which you may submit to your insurance company for possible reimbursement.

 

Please note: Reimbursement is not guaranteed and depends on your individual plan.

GOOD FAITH ESTIMATE NOTICE

Under the No Surprises Act, clients who do not have insurance or who are not using insurance have the right to receive a Good Faith Estimate for the expected cost of services.

 

You will receive a written Good Faith Estimate before your first session, outlining the total estimated cost of speech, language, or feeding services. This estimate is based on your individualized care plan and can be requested at any time.

TELEHEALTH DISCLAIMER

Pure Connections Therapy offers virtual therapy services using secure, HIPAA-compliant telehealth platforms. While teletherapy may not be appropriate for all clients or all situations, we are committed to ensuring a safe, effective, and confidential online experience for those who choose this option.

SCOPE OF PRACTICE

Evaluations and treatment plans are individualized and based on each client’s specific needs. While we are committed to providing evidence-based, compassionate care, progress may vary, and therapy outcomes cannot be guaranteed.

LICENSING & CREDENTIALS

All services are provided by a licensed and ASHA-certified speech-language pathologist in the state of Texas. We meet all continuing education and clinical standards required for professional practice.
